REMINDER REGARDING USARS PAPERWORK: Incomplete applications will not be processed. Any application that is incomplete will be placed on hold until the outstanding items are received. The following are the most common reasons that an application may be placed on hold:

1. The applicant is a new member and failed to submit birth verification (copy of driver’s license, passport, or birth certificate).
2. The applicant failed to sign the bottom of the application.
3. Incomplete or no payment information was provided on the application.
4. The charter for the club/league is not active yet and individual memberships cannot be processed until that is done.

Bring Your Own Sponsor: $200 When a team brings their own sponsor and provides their own jerseys, there will be an additional mandatory advertising fee for the sponsor.
